Often the wear indicator doing exactly its job. Occasionally hardware or glazing, which is why it is looked at rather than assumed.
Metal against metal. The pad material is gone and the disc is going with it, and the longer it runs the more of the job becomes discs.
Felt through the pedal or the wheel, usually disc run-out or uneven pad deposits, and most obvious from higher speed.
The car takes more road than it used to. Glazed pads, tired fluid or a caliper not applying evenly.
Air or moisture in the fluid, or a leak somewhere in the system. This one is a same-day job rather than a next-service job.
A caliper that is not applying or releasing evenly, and it will wear one side out while it does it.
Level, wear sensor or the ABS and stability system. All three are readable, and one of them is a leak.
On an electronic parking brake this is a motor, a cable or a control fault, and it usually appears right after a brake job done without the service mode.

On a car with an electronic parking brake, not safely. The rear pistons have to be retracted electronically, and forcing them is how the motor fails. The tool takes a minute and saves a part that costs many times the pads.
It is a controlled series of stops that transfers an even layer of friction material onto the disc. Done properly the brakes are quiet and smooth from the start. Skipped, you get the judder and squeal that people assume is normal after a brake job.
